How to Work With Hospice: Refer a Patient
To refer a patient to Hospice, call the Hospice office at:

- Avondale: (602) 678-1313 (Office)
When you refer a patient we will respond to your referral immediately. We will establish contact with your patient as soon as practical (within 1 hour of your call is our Goal). We will assess the patient for hospice appropriateness. This is most often accomplished in the discussion between you and our Admission Staff during your referral call. Once we have established that your patient meets hospice admission criteria, we set up an admission visit with the patient and the family to review our services and provide informed consent. When the patient and family agree to accept our services we admit them and work with you to establish a Plan of Care. Our services begin at admission.
